Client Agreement and Terms

Thank you for choosing Hannah Elizabeth Holistic Health Coach for your wellness journey. Please carefully read and agree to the terms outlined below. This agreement must be filled out and signed at least 48 hours before our first coaching session.

1. Coaching Structure:

  • The coaching commitment is for 3 months (12 weeks). Sessions are scheduled weekly; they alternate weekly between video call and phone call. The first session is a full hour, and subsequent sessions are 45 minutes.

  • Coaching can begin at any time within a month, defined as a 4-week period, not necessarily a calendar month. At the completion of the 3-month period, clients can opt for a month-to-month arrangement or commit to another 3-month term to address further health and behavior changes.

  • Scheduling is open for 3 weeks at a time. At the end of each week, another week will become open for scheduling. 

  • Client is responsible for self-scheduling. 

2. Business Hours

  • Coach is available via email Monday through Friday, excluding major holidays. You can expect a response within 1-2 business days. 

  • Phone calls and video calls will be a part of the coaching program; however, all unscheduled communication will be conducted over email. 

3. Rescheduling Policy:

  • Clients must request rescheduling of virtual video/phone meetings at least 48 hours in advance.

  • Failure to provide adequate notice will result in a $100 rescheduling fee.

  • Rescheduled sessions will be accommodated within the same week or the following week, based on availability. If there is no availability in those weeks, the client forfeits that session.

4. Fees:

  • The full 3-month fee of $2295 is due before the first session.

  • This fee includes twelve 45-minute virtual coaching calls (alternating between video and phone each week), email support, and phone support at the coach's discretion and availability throughout the business week. 

  • Payment is accepted through PayPal or Venmo.

  • A month-to-month payment plan is available at a rate of $815 per month. Payment for the first month is due before the first session. Subsequent monthly payments are due on the same day each month.

5. Cancellation and Refund Policy:

  • Once coaching has begun, the full 3-month fee is non-refundable.

  • In the event of client cancellation before the commencement of the initial coaching session, a refund will be provided, minus a $150 administrative fee.

  • In the event of an unforeseen life event that necessitates the discontinuation of coaching, the coach, at their discretion, may allow the payment to be deferred to a later time within the subsequent 12 months.

6. Nature of Coaching:

  • Coaching is not a substitute for medical or psychological care. Clients are responsible for their own well-being and results. In the event that you feel the need for professional medical help, counseling or therapy, it is your responsibility to seek a licensed professional.

  • Health coaching results are not guaranteed, and you enter into coaching with the understanding that you are responsible for creating your own results.

  • As part of my training, you are working with me on a volunteer basis for the purpose of creating a structure to identify and achieve your health and well-being goals.

7. Confidentiality:

  • I recognize that in the course of our work together, you may provide sensitive personal, health, and financial information. I commit not to use any information for personal benefit, disclose, or communicate it to any third party.

  • I will not divulge that you and I are in a coaching relationship without your permission.

  • All information shared will be held confidential unless there is a concern for physical danger to yourself or others, in which case, legal authorities will be informed to take protective measures.

8. Agreement Confirmation:

  • By signing and submitting this agreement, you confirm your understanding and agreement to the terms outlined above.
